KINDERGARTEN REGISTRATION
2025-2026
London City Schools is thrilled to prepare for the arrival of the Red Raider Class of 2038! If you have a child who will be 5 years old on or before August 1, 2025, they are eligible to enroll in kindergarten for the 2025-2026 school year. Our registration week will be March 31- April 4
Enrollment is a two-step process. Your student will not be enrolled until both steps have been completed.
Step 1
Please visit Final Forms at https://london-oh.finalforms.com/. Click on Parent *New Account* to register your child and start the enrollment process. You will create only one parent account to add all of your children. These forms must be completed prior to your in-person registration meeting and in order for your student to attend school.
Step 2
To set up your appointment during the week of March 31-April 4, click HERE. You must bring in the required documents listed below. Your child does NOT need to be present during the registration process. Screenings will be completed at a later date.
Required Documents are as follows:
1. Parent Photo ID
2. Official Birth Certificate - Not the certificate from the hospital with footprints.
3. Proof of Residence-2 Proofs of Residence (lease, current utility bill, property tax statement, mortgage statement)
4. Custody Documents - If applicable
5. Immunizations
6. Current Physical
IMMUNIZATIONS AND PHYSICAL
The State Department of Health and London City Schools require the following:
MMR – Must be given to the child on or after first birthday (2 doses)
Dtap – 5 doses (Basic series and a booster after age 4)
Polio Vaccine – 4 doses (Basic series and a booster after age 4)
Hepatitis B – 3 doses
Varicella (Chicken Pox) – 2 doses after first birthday
A physical examination will be required prior to school starting in August. Physical examinations done within 1 year prior to school starting will be accepted. If your child has had a physical examination done in August 2024 to present, please bring it with you to registration. Please note, if a student’s immunizations are not up to date and a current physical has not been turned in, your child will not be permitted to attend school after September 2, 2025. If you are unable to pay for your vaccinations, please call the Madison County Health Department for assistance. If your doctor’s office requests a physical form, please click HERE.
